To help refine materials being developed for a new initiative, the United Hospital Fund of New York's Families and Health Care Project has engaged a range of family caregivers in a series of focus group sessions. The new initiative is creating the materials to guide caregivers and the family members they assist through transitions into and out of different health care settings—from hospital to home, for example. Complementing this effort, the Fund is also working with hospitals, skilled nursing facilities, and certified home health agencies to help them better support family caregivers, and in the process improve patient safety and quality of care.
