"Palliative Care from the Outside In:  How a Community-Based Hospice Can Support the Hospital in their Palliative Care Initiative," is the title of the Thursday, December 6 audio conference from 1 to 2 pm CST, offered by Supportive Care Coalition.  This conference, featuring CHRISTUS VNA Hospice and Palliative Care, will share ways to relieve the hospital’s burden of creating a separate and unique Palliative Care Team by working with a community-based hospice that supports palliative care as well as the terminally ill.  This audio conference will demonstrate the utilization of the hospice core team in providing expertise in the palliative care concept, develop the RN liaison model to assist hospital case management, demonstrate the use of the home care model assists in patients’ through-put, relieving emergency room and intensive care census; ways to apply JCAHO standards to enhance compliance, and ways to support the spiritual needs of the palliative care patient.  Christina Mc Clean, RN, MSN, CHPN, Director of CHRISTUS VNA Hospice and Palliative Care in San Antonio, TX, will make this presentation at the Supportive Care Coalition's quarterly board meeting in Dallas, TX on Thursday, December 6, 2007, at 1 pm CST.
