Coalition Audioconference Opportunity

Registration is open for the Supportive Care Coalition's free audioconference/Web cast, The Personal Health Record's Role in Supportive Care - The Link for Patients, Families and Caregivers, to be broadcast on Thursday, November 2 at 9 a.m. PST

Broadcast from the Coalition's quarterly board meeting, the offering will showcase how an intranet-based network connects provider communities, patients, their families and caregivers to enhance end-of-life care. The current state of healthcare demands a new model of care to support chronic disease management and patient partnership in care. The Whatcom Health Information Network (HInet), and the Pursuing Perfection Program in Whatcom County, Washington has accomplished just that - a private intranet system that connects provider communities to one another and facilitates communication with patients, families and caregivers to support a shared care plan. 

Lori Nichols, director of the Whatcom Health Information Network, LLC (HInet) and Pursuing Perfection Program, owned by St. Joseph Hospital, a member of PeaceHealth, will share how this connection can make a difference in end-of-life care.

To participate in the audioconference, registrants must have audio access (speakerphone if multiple participants are in the room), and a computer with internet access to facilitate a "walk-through" of a sample plan online.
