The current state of healthcare
demands a new model of care to support chronic disease
management and patient partnership in care. The Whatcom
Health Information Network (HInet), and the Pursuing
Perfection Program in Whatcom County, Washington has
accomplished just that - a private intranet system that
connects provider communities to one another and facilitates
communication with patients, families and caregivers to
support a shared care plan. Participants will learn how
this connection can make a difference in end-of-life care.

The
Changing Face of Advance Care Planning: Caring Conversations
As One Example

This presentation provides a brief historical overview of
advance care planning, information about the paradigm shift
from a legal transactional model to a communications model
of advance care planning and what has motivated this change,
exposure to Caring Conversations as a model developed by the
Center for Practical Bioethics and be informed of several
other transactional models, awareness of how Faith
communities through the Compassion Sabbath Program have been
engaged in advancing the new ACP model, information about
National Healthcare Decisions Day, April 16, and how to
become involved in it.
