The American Hospital Association offers a downloadable brochure, Put It in Writing, that provides basic facts about advance directives and encourages patients to explore their preferences for care at the end of life.  

The site also features a printable, wallet card that alerts healthcare workers that the individual has talked to his or her family about advance directives and provides contact names and numbers.
