The American Hospital Association offers a downloadable brochure that provides basic facts about advance directives and encourages patients to explore their preferences for care at the end of life. 

The site also features a printable, wallet sized card that alerts healthcare workers that the individual has talked to his or her family about advance directives and provides contact names and numbers.
