Successful advance care planning cannot depend only on motivating individuals to do advance planning, asserts the Respecting Choices® program of Gunderson Lutheran Medical Center in Lacrosse, Wisconsin. It also requires a system of practices, policies, and staff education so that advance care plans are obtained, stored, retrieved, updated, and finally, respected at the proper time.
The Respecting Choices® program offers a toolkit designed to help organizations assess how their current system works (process and outcomes) and develop a more effective system to respect a patient's recorded plans for future medical care.
