Conversations about advance care planning can be daunting even when the health care provider and client share the same language. When patients and families are not native English speakers, it can become even more difficult to elicit and honor the wishes of the patient.
The Spiritual Care Department of Northridge Hospital Medical Center in Northridge, California, has prepared and translated a brochure entitled "Planning for the Future." The brochure briefly covers common issues and questions in advance care planning. It is an especially useful tool for patients who are not English speakers and is made available here with the permission of the Northridge Spiritual Care Department.
