The Choices Bank is a free community service from St. Patrick Hospital and Health Sciences Center in Misssoula, Montana. It makes your advance directive available anytime, anywhere with the level of privacy you choose. There is no charge for depositing your advance directive, viewing it, printing it, or for depositing revised versions of it.
