Pallilative and End-of-Life Care Programs Include:

Provena Home Care and Hospice is working in partnership with Provena Saint Joseph's Hospital, Elgin to develop a palliative care consult team which will concentrate on hospital referrals with emphasis on appropriate discharge planning to home care, nursing facility or hospice.

Provena Saint Mary's Hospital, Kankakee, and Provena Saint Joseph Medical Center, Joliet, are partnering with community hospices to create both inpatient and outpatient palliative care and services.

Other ministries exploring palliative care program development include Provena Pine View Nursing Home, Saint Charles, and Provena Covenant Medical Center, Urbana.
