End-of-Life program highlights include:

OSF Hospice that services all six of the OSF regions.

OSF Home Health's development of a region-wide bridge program to assist patients facing a serious illness who may not qualify or may not be ready to accept hospice.

An in-patient Palliative Care Program at St. Anthony Medical Center in Rockford.

A Model for Supportive Care, designed by a designated team charged with exploring end-of-life issues and how to provide improved services to patients.  After a thorough research and review of evidenced-based practices, the following was approved by the Board of Directors:

Supportive Care Model that crosses the continuum of care adopted
1. Establishment of a Governance Council  to assure this model is adopted and implemented throughout the healthcare continuum of care
2. Development of an infrastructure to assure implementation of programs within each region

Three new program recommendations adopted
1. Each OSF hospital to have an inpatient palliative care program plan developed by October 2007
2. Establishment of regional teams following the IHI principles for collaborative work.  These teams will test change at a micro system level, collaborate with one another, and implement standardized care models.  Their first assignment is to develop and implement a standardized Advance Care Planning process that will be used by caregivers across the continuum of care.
3. The Home Care Division will implement a pilot that will focus attention on coordinating services and helping patients with chronic progressive and/or terminal illness 'walk the walk' and maneuver through the OSF system.  This model will be built following many of the principles established by the Coalition's CALL Care demonstration project. 

OSF HealthCare recognizes the model will continue to evolve as the culture adapts to this change in care delivery.
